I understand that the K8彩乐园 (UNA) does not require me to participate in campus recr吃ion (包括 but not limited to intramurals, 市外的参与, 俱乐部体育, 物理运动, 重量和心血管训练, 教学计划, fitness 程序s and classes and any other 程序 and services sponsored by UNA娱乐, 包括相关的旅行和UNA户外探险中心.  My participation in these activities, events, 程序s and sports is completely voluntary. THIS DISCLAIMER AND EXCLUSION INCLUDES ANY AND ALL EXPRESS OR IMPLIED WARRANTIES OF FITNESS, 适合某一特定用途, 适销性, 以及所有其他明示或暗示的保证.

I do hereby further declare myself to be physically sound and suffering 从 no condition, 障碍, 疾病, 虚弱, 或其他疾病，使我无法参加UNA俱乐部的运动, 校内的运动, 或者任何活动和项目, UNA的各种设施, 或使用设备和机器. I hereby acknowledge that it is recommended that I obtain a physician’s approval for my/our participation in an 锻炼/fitness activity or in the use of 锻炼 equipment and machinery. I also acknowledge that it has been recommended that I have an annual or more frequent physical examination and consultation with my physician as to physical activity, 锻炼, and use of 锻炼 and training equipment so that I might have recommendations concerning sports and fitness activities and equipment use. 

I hereby understand the importance of having a physical examination and physician’s approval prior to participating in any 锻炼, 为了我自己的健康和安全健身或运动项目, 我在此为我的决定承担全部责任, 以及我/我们在上述活动中的参与情况, 在我的活动中使用设备或机器.

医疗保险:

I acknowledge that UNA does not carry health or accident insurance to cover for these voluntary activities, and I understand and agree that coverage and payment for medical tr吃ment or injury related 成本 are my responsibility or the responsibility of my legal guardian. 通过参与, I acknowledge that I have adequate medical coverage for any Activities I will or I am participating in. 

紧急处理:

I hereby consent and give my permission to be tr吃ed for emergency medical care and first aid by a medical facility and/or university personnel. The authority and release granted in the preceding sentence shall include the right (at the sole discretion of UNA) to place me, 自费, 如果被认为是紧急情况，无需进一步同意, in a hospital or with an emergency medical provider for emergency medical services and tr吃ment, 或者如果没有现成的急救人员, 与当地医生或医疗机构进行治疗, to include any first aid tr吃ment offered or implemented by the K8彩乐园 工作人员. 我同意承担与此治疗有关的所有费用，包括运输费用.
